She assumed that Hillary Clinton would win the 2016 Presidential Election, and because of her close relationship with the Clintons (Bill nominated her for the Supreme Court in 1993), RBG wanted her seat to be filled by Hillary. Trump apparently told the GOP caucus not to touch Medicaid, which is a problem for the most conservative members.
The issue is entirely political. The GOP does not want to cut Medicare because everybody ends up with it and it would kill them even with their base.
But Medicaid is for poor people. Much narrower base. Still, Trump is smart here because the imagery of cutting medical care for poor people is a really, really bad look.
