Honestly? We need to beat South Carolina.. but
I'd be excited with just giving Ole Miss a good game. We shouldn't really be able to hang with them, so making it a close game with a great atmosphere (for recruits watching) would be a very good sign for us at this point.
We'll see, they have ballers at QB and RB, but their defense isn't great and our defense is far better than I thought they would be this year. It should be a shoot out imo

I asked about early onset Alzheimer’s (he is in his forties) but they’re pretty sure that isn’t it. Liver failure can cause problems very similar to dementia but it’s usually temporary and goes away when the ammonia goes down. With a transplant symptoms of HE usually go away completely within a year. He’s never had serious mental health issues before all of this but because of the HE, liver failure and cancer I’m afraid he might be developing something similar to schizophrenia. He sees things and talks to people as well but normally it goes away with a heavy dose of lactulose.

This time it’s not going away and the last time we went to the hospital it was because he was symptomatic and talking to people/seeing things. I was convinced his ammonia would turn out to be high but it was only slightly elevated. He cleared up and I didn’t think much of it until this episode which has been going on for over a week and getting progressively worse despite regular lactulose.

Did your grandmother have schizophrenia before the dementia? My granny also suffered from it but for the most part she was more stuck in the past and had trouble remembering day to day things. Regardless I’m so sorry to hear that you and your family are having to go through that. It’s a horrible illness.
To my knowledge she did not have a past history of it. I unfortunately haven't been able to see her since shortly after the diagnosis. At that time it was just repeating things and asking the same question several times. I've only been told the stories of how it's changed since. She's surprisingly sharp in it though. She just accepts that she doesn't know who you are but that you make her feel good to be near. If she thinks you're a stranger she won't tell you about the people she's seeing and that are talking to her. If she's told you are family then she will talk to everyone in the room (non-existent ones included) and tell you about all of them. Then, for about 30 seconds every 2-3 hrs she knows who and where she is and will reassure you that things will be okay and that she's doing her best and not suffering. Gets out everything she can before it takes over again. Having it described to me was like the script of a horror movie about someone fighting a possession.

It's sad, but she doesn't seem to be suffering through it anymore. She was scared when it first started but now she's comfortable with it it seems. My grandfather has taken care of her through all of it and has good humor about it. My aunts have a schedule with him where they take care of her for periods of time to let him get out of the house.

Probably more than I needed to share and off topic... your situation though - Praying for y'all. I have no experience with liver issues so there's not much I could help with than that. I hope a transplant comes soon!

I don't know if he has asterixis (I'm going to have to study up on that one) but he has lost a massive amount of muscle from all of this and definitely has HE. I didn't think it possible to atrophy so quickly. He has trouble with coordination and standing completely upright. His billirubin is definitely high (bright orange to blood red). What is odd is that when he's been symptomatic before it's largely been a matter of increasing his lactulose and making sure he was having enough BMs but this time it's as though he's not responding at all to the lactulose beyond having BMs. Normally, when it's HE his symptoms go away with an increased dose.

That said, he has had mirror symptoms of HE when he's had sodium/potassium imbalances and a few years prior when he had an acute episode of kidney failure. I'd have to check my notes but I think his last brain scan was only about 2 months ago and there was nothing there.
I’m really sorry to hear all of that. Asterixis is a reaction some people have with HE. You can have him extend his arms out and cock up his hands backward like stopping traffic and hold it. I’m sure you can YouTube this.

You can lose muscle mass quickly with not moving around much but cirrhosis is a catabolic state that just eats of muscle very quickly.

Confusion in liver patients doesn’t always mean HE. Like you mentioned prior, electrolyte abnormalities are very common in cirrhosis patients. Especially hyponatremia. Does he have ascites? They can become spontaneously infected and can have an array of symptoms.

How high is the bilirubin? Sometimes people can go into an acute liver failure which would be alarming.

I really would make sure you are seeking medical care with labs at a minimum

Has to switch back. Couldn’t shake the feeling that the FBI was trying to locate me. 😳

Already had my phone and browser on dark mode, so the VN version didn't change much. But the 1st week or 2 after I switched, it was freaking me out.

Tried light mode yesterday - just to see the full comparison for VN and now the light mode had me all messed up.
Apparently I adapt hard but like everything else, takes me a bit to get there, haha.
