My department green lights an IT deployment without risk management in place for downstream impacts to data on which certain operations depend. Our AVP's outlook is to see what breaks, then fire drill any fixes. Oh joy, oh great, great joy...

Seriously?

I hope everything is backed up off-site 38 ways from Tuesday. And I shouldn't say it, but if things go wrong, the AVP should be looking for new employment. You just don't do that.
