Most, I’d say 99% or more of all automobile crashes (calling them accidents is misleading) are due to driver error. When you make a mistake and kill someone else you should be held accountable, if we did that maybe we would get better drivers.

I have been driving for over 50 years, with 10 on the road as a salesman in a box truck, and have never hit another vehicle. Deer is another story. I have avoided countless wrecks by paying attention.
