He's almost certainly doing a schtick, but it's so unfunny even if I'm certain he's not mentally ill. I just don't understand how anyone can laugh at it unless they're just feeling so good about themselves because they aren't as bad as he is.

Two things: Peter Sellers first two Pink Panther movies were the better, more subtle humor (that’s just my opinion but you have to watch all those movies). When watching Sam Kinison, keep in mind that he was once a Baptist preacher with the same approach!
