Because she is devastated by the harming of zombies but has no problem hurting people? The insanity doesn't seem consistent to me.
Consistency isn't a word I'd use with anything regarding insanity
What about OCD? I've known Schizos who are very consistent about the things that make them unravel (like people dressed in orange).
What's the girl's diagnosis, anyways? I'm not trying to act like an expert on psychology, but I just don't know what kind of psychosis would have you defending a zombie and taking an innocent life in the same day.

What's confusing is that her mental illness was portrayed in a half cocked manner. She apologized to Carol profusely for pointing a gun at her, which confirms she understands that weapons kill and should not be used on loved ones. Yet, moments before she casually uses a knife to murder her little sister.
A more consistent response to Carol, in my opinion, would have been lizzie extending the invite to Carol and Tyrese to change them as well; walking slowly and gleefully towards them with that knife
