run your xbox directly from your modem and do not use a router. You need to change you settings on you connect screen. I think if you do a test you can also change your NAT to open.

Depends on the brand of your router and how strict its hardware firewall is.

You can usually also open the specific ports that xbox uses. Post what brand/model router you have.

Either way you will likely need to give your xbox a static ip address.

I've been having trouble connecting to my brother for 3-4 months... he updated his router's firmware today and it now magically works!

I have a Linksys router. Thing is that when I had comcast cable it worked with no problem. When I moved and now have Direct TV with Bellsouth fast access it is limiting my NAT to Strict.
