It doesn't matter at this point what the real problem is, because ultimately it IS a coaching problem. 4 games later we're still having this issue, and if the coaches still allow it to happen then it's now their fault. It's the same thing when you're a manager and your employee keeps making the same mistake. The employee is still the one making the mistake but as the manager it's now your fault for letting keep happening.
There's no way you can shrug your shoulders and proclaim it's the players' fault 4 freakin' games later.
