I don't know how to get you the sample one. It's a print out I was given. I can explain how it's laid out as best I can.

Do you have access to a scanner? I would think your library has one. Or, an iPhone? A very underutilized function of the iPhone is taking photos of papers or important documents. It's extremely easy to pinch and zoom to read the fine print on the phone. It's a great way to keep track of info, easy to save in iPhoto, and easily accessible on the phone.
