The Merritt Parkway in Connecticut has those. Nothing like hopping out into 75mph traffic from a dead stop.

I haven't been in Knoxville in some years but there is (was) an exit onto I-40 W that had you entering into the left lane of traffic.

The worst are the ones that slow down so much to enter the deceleration/turning lane that they actually have to speed up once they get there. That's incredibly coomon here in Fayetteville
