Mass Effect 3 May Require You to Rebuild Shepard - Xbox 360 News at IGN
IGN is receiving reports from several readers that Mass Effect 3's character import system requires them to rebuild Commander Shepard's face.
Players who have used the same Commander Shepard since Mass Effect 1 are apparently encountering the problem. If you built a Commander Shepard in the first Mass Effect and played through Mass Effect 2 without modifying the character, Mass Effect 3 will restore him to the default settings. Players who modified their character at all in Mass Effect 2 seem to be uneffected.

Importing a Character in Mass Effect 3
On Twitter, Mass Effect producer Michael Gamble confirmed the issue, and we've reached out to EA to find out if a fix is planned. This is the second issue with character importing reported today, coming shortly after the news that Mass Effect 2 cloud saves will not work with Mass Effect 3.
