Did anyone else get one of these in the mail recently? I'll probably fill it out and send it in. Just curious what others thoughts are as to the following questions...

Yeah... pretty much. Unfortunately I think we have quite a few members of Congress suffering from Alzheimer's.

I received it in the mail with the only option for submittal being a paper survey and prepaid envelope. The letter that came with it doesn't mention an option for an online submission.
