i dunno if this will go anywhere, but i hope... there are of course some obvious ones.. but maybe some people have forgotten about... or just really unlucky sports moments, smaller chokes, anything?

i was thinking that patrick serna kicker (i think tha was his name) missing PAT's against lsu 2 years ago

Bar none, that's the worst I can remember. The saddest part was that he had about 3 chances to stop choking while he was choking . . . and still choked.
