But seriously, what is going on? How does an entire time get the yips? Do the players ever have any comment after these games taking responsibility and whatnot? Obviously I don't follow all this super-closely, so any informed commentary would be welcome.

Even if it's a coaching issue, 28.4% free throws? Surely free throws are pretty much gimmes.
