Yeah, I checked it as well. I may be wrong but unless you have ESPN GAMEPLAN I don't think you will be able to find it with DirecTV or Dish?

Not wrong but Comcast has an hd channel of it. Somewhere in the 1,000-1,400's

Not 24/7 but CSSHD does broadcast on channel 828. Keep checking the guide for program availability. Selected times are broadcasted. I've caught a couple games this season.
