In my opinion a lot of it is their poor tactical use mixed with poor training/filling out crews with soldiers who were never trained on tanks.

Saw a tweet last month where a T-72B had three Lieutenants crewing it all from non-combat jobs.

Russia did the same thing in Chechnya and their tanks got obliterated, now the casualties are ramped up because they're fighting a better trained and equipped force.
